Patients name, address, telephone,social security number emergency contact. Name, address and telephone of the patients insurance carrier; insurance policy group and contact number.
Insureds employer company name, address and telephone.
Name, address, and telephone of insured if other than the patients relationship to the insured.
The insurance company will be contacted for the benefit verification.
The patient or responsible party will then be advised of the benefits.
Following the requirements of your insurance company, they will work with them making every attempt to obtain maximum reimbursement.
Cash Accounts Cash accounts and payment terms are based upon the length of stay and are scheduled with the patient or responsible party prior to admission.
Visa, Discover, MasterCard and American Express are accepted.
