About Phoenix House

The Phoenix House Inc caters to those with substance abuse issues for young adults and adults.


In order to receive treatment, the following is required:
1)    Standardized Psycho-Social Assessment with complete DSM-IV Diagnostic Codes
2)    Priority Placement (Code3)     Consent for Release of Confidential Client Information (Universal Form)
4)    Assessment Update (Progress Summaries required if Assessment is more than one month old)


There is a sliding fee scale based on income and other factors, payment assistance, cash, or self-payment.
Admission fee is $125.00, however no one is turned away based on their inability to pay.
Please contact the facility for any further information that is needed.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free treatment from some substance abuse centers, they use a sliding fee scale based on your income. When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
