About Fall River Men's Program
The Transition House provides transitional housing combined with supportive services to ten disabled homeless person in the City of Fall River.
Case Management services provided at the Transition House connect clients to housing, employment, entitlements, behavioral health, and and array of other services.
Clients become engaged in all supportive services needed and as a result, successfully transition into the community and experience housing stability and improved health and quality of life.
Admission criteria includes must be 18 years or older, Homeless in the city of Fall River and presenting a history of multiple relapse episodes, diagnosed with co-occurring or co-existing disorders, must meet HUDs income guidelines, willing to authorize a CORI check, CORI report must be without warrants, alcohol and drug free and medically and psychologically stabilized.
